The mental health and wellbeing of children and young people is shaped by experiences in different aspects of their lives; including at school, at home, relationships with family and peers, interactions with digital technology and within broader mental health systems.
Personalised care aims to improve health and wellbeing outcomes by approaching an individual’s care through exploration of alternatives and individually significant options. This can include community-based options and can include social prescribing. But what is social prescribing? Social Prescribing is a process that enables health and social care professionals, community development workers and others to refer someone to a Social Prescribing Link Worker in order to talk about the issues they are facing and find the right services (often in their local area) to support and empower them. Social Prescribing Link Workers help address people’s needs across a number of areas including support with physical activity, diet and nutrition, housing, mental health etc.
For example, social prescribing may suggest: discovering and spending time in green spaces in your local area, the use of outdoor gyms, spending time around animals and getting involved in a farm or free counselling to support your wellbeing.
